In short
- The UK Mounjaro SmPC lists delayed stomach emptying as uncommon (up to 1 in 100) and does not list ileus
- Constipation is very common (more than 1 in 10) in people using Mounjaro for weight management
- A 2023 JAMA study found 4.22 times the risk of bowel obstruction with older GLP-1 medicines against another weight loss drug, with small absolute numbers
- No wind, no stools, a swollen tummy and vomiting together is an emergency
- Previous tummy surgery, a hernia or Crohn's disease raise the general risk of a blocked bowel
Bowel obstruction and ileus are rare on Mounjaro, but both are emergencies. Ileus has been reported after marketing with medicines in the GLP-1 class, and tirzepatide slows the gut in the same way. The UK SmPC does not list ileus. If you pass no wind or stools and you also have a swollen, painful tummy and vomiting, call 999 or go to A&E.
What ileus and bowel obstruction mean
Food moves through the bowel because the muscle in its wall squeezes in waves. Two things stop the flow.
- Mechanical obstruction is a physical block. Scar tissue (adhesions) from old surgery, a hernia, a narrowing from Crohn's disease or a growth can close the tube.
- Ileus, sometimes called paralytic ileus, is a block with no physical cause. The muscle stops pushing, so contents stay where they are.
Both produce the same problem. Gas, fluid and food build up above the hold-up, the bowel stretches, and the body starts to vomit what cannot pass. Without treatment the bowel wall can lose its blood supply or tear. Hospital teams treat both conditions as urgent.
Why Mounjaro is linked to the bowel
Tirzepatide acts on GLP-1 and GIP receptors. One effect is a slower stomach. The SmPC states tirzepatide delays gastric emptying and lists delayed emptying of the stomach as uncommon, affecting up to 1 in 100 people. The delay eases over time.
The rest of the gut slows too. Constipation is very common in people using Mounjaro for weight management, which means more than 1 in 10. Most of these cases are mild and settle with fluids, fibre and movement. The constipation page covers what to do.
Ileus sits at the far end of the same scale. Reports of ileus with GLP-1 medicines led regulators in some other countries to add a warning to their product information. The UK Mounjaro SmPC and leaflet, as checked for this page, do not mention ileus or intestinal obstruction.
What the research shows
Large trials of tirzepatide were not big enough to measure a rare event such as obstruction with any confidence. The best data come from older GLP-1 medicines.
A 2023 study in JAMA by Sodhi and colleagues looked at insurance records for people using semaglutide or liraglutide for weight loss. The researchers compared them with people taking bupropion-naltrexone, a different weight loss drug. The GLP-1 group had 4.22 times the risk of bowel obstruction (95% confidence interval 1.02 to 17.40). The wide interval reflects small numbers.
| Medicine group | Bowel obstruction cases per 1,000 person-years |
|---|---|
| Liraglutide | 8.1 |
| Semaglutide | 0 |
| Bupropion-naltrexone | 1.7 |
Tirzepatide was not part of the study. The figures show a raised relative risk with a low absolute rate. Most people on a GLP-1 medicine will never have an obstruction.
Who carries a higher risk
The general risk factors for a blocked bowel apply on Mounjaro as well. Tell your prescriber before you start if any of these apply to you:
- Previous surgery on the tummy or pelvis, including a caesarean section, appendix removal or bowel surgery
- A hernia in the groin or the abdominal wall
- Crohn's disease or diverticular disease
- A past episode of bowel obstruction or ileus
- Long-standing severe constipation
- Regular opioid painkillers such as codeine, tramadol or morphine, which slow the bowel further
- Recent surgery, because ileus often follows an operation
Mounjaro also matters around anaesthesia. The surgery page explains what to tell the hospital.
The symptoms to watch for
An NHS trust leaflet on bowel obstruction lists these signs:
- Cramping tummy pain which comes and goes in waves
- Feeling sick, then vomiting
- Bloating or a tight, full tummy
- Heartburn or acid reflux
- Not passing wind or stools
- Loud gurgling from the abdomen
The pattern matters more than any one symptom. Ordinary constipation leaves you able to pass wind and keep fluids down. With an obstruction both stop. Vomit may turn green, then brown and foul-smelling as the hold-up continues. Pain often begins as cramps and becomes constant.
How this differs from gastroparesis
Gastroparesis is a slow stomach. You feel full after a few mouthfuls and you may vomit undigested food hours after eating, but wind and stools still pass. Read the gastroparesis page if fullness and late vomiting are your main problems.
What happens in hospital
Doctors examine your tummy and arrange blood tests and a scan, usually an X-ray or a CT. Many people with ileus or a partial block recover with bowel rest. Staff stop food and drink, give fluids through a drip and may pass a thin tube through the nose into the stomach to drain fluid and gas. A full or twisted obstruction may need an operation.
Tell the team you use Mounjaro and give the date of your last injection. A single dose lasts around a week in the body, so its effect on the gut continues for several days.
Steps to lower your risk
- Treat constipation early. Do not wait a week with no stool before you act.
- Drink 6 to 8 glasses of fluid a day, more in hot weather or if you vomit. See nausea and vomiting.
- Build up fibre slowly from fruit, vegetables, oats and pulses.
- Walk every day. Movement helps the bowel push contents along.
- Ask your prescriber about staying on your current dose if constipation is severe.
- Avoid opioid painkillers unless a doctor advises them.
When to get help
Call 999 or go to A&E if:
- You have not passed wind or stools and your tummy is swollen and painful
- You keep vomiting, especially green, brown or foul-smelling vomit
- Severe tummy pain does not go away, or spreads to your back (possible pancreatitis)
- Your tummy is hard and tender to touch, or you feel faint, cold and clammy
Call 111 or see a GP today if:
- You have not opened your bowels for 3 or more days and you have cramps or bloating
- Laxatives have not worked after 2 days
- You are vomiting and cannot keep fluids down for more than a day
- You have tummy pain and a known hernia, Crohn's disease or past obstruction
Mention at your next check if:
- Constipation is a regular problem on your current dose
- You have had abdominal surgery since you started Mounjaro
- You have started an opioid painkiller or another medicine which slows the bowel
Do not inject your next dose while a blocked bowel is suspected or being treated. Ask your prescriber before you restart. Report any suspected bowel obstruction to the MHRA Yellow Card scheme. Mounjaro carries the black triangle for additional monitoring.
Questions people ask
Can Mounjaro cause a bowel obstruction?
A bowel obstruction on Mounjaro is rare. Ileus has been reported with medicines in the GLP-1 class, and Mounjaro slows how fast food moves through the gut. The UK SmPC does not list ileus or obstruction, so the size of any risk for tirzepatide is not established.
What is the difference between constipation and ileus?
With constipation you pass stools less often or with effort, but you still pass wind and the tummy settles with fluids, fibre or a laxative. With ileus or an obstruction nothing passes at all, the tummy swells and hurts, and you start vomiting. The second picture needs emergency care.
What are the first signs of a blocked bowel?
Cramping pain in waves, a bloated tight tummy, feeling sick and no wind or stools are the early signs. Vomiting often follows, and the vomit may turn green or brown. Call 999 or go to A&E if these come together.
Should I take my next Mounjaro dose if I think my bowel is blocked?
No. Do not inject while you are waiting to be seen or while doctors are treating a suspected obstruction. Tell the team you use Mounjaro and when your last dose was, and ask your prescriber before you restart.
Who is more likely to get a bowel obstruction?
People with scar tissue from previous tummy or pelvic surgery, a hernia, Crohn's disease, a past bowel obstruction or long-standing severe constipation carry a higher general risk. Opioid painkillers slow the bowel further. Tell your prescriber about any of these before you start.
