In short

  • Store unstarted pens in a fridge at 2C to 8C. The leaflet gives no out-of-fridge allowance before first use
  • A frozen pen must be thrown away, even if it looks normal after thawing
  • GPhC guidance asks online pharmacies to assess delivery methods and packaging for refrigerated medicines
  • GPhC guidance asks for parcels to be tracked and monitored to make sure they reach the right person
  • Online pharmacies must display their GPhC registration number, which you can check on the GPhC register

Mounjaro pens travel from the pharmacy in an insulated box with cooling packs, sent by tracked courier, usually for next-day delivery. The packaging keeps unstarted pens at 2C to 8C for the planned journey, and the job passes to you at the door. Be in to take the parcel, check the packs are still cold and the pens are not frozen, and put the pens in your fridge straight away. If anything looks wrong, do not inject. Contact the pharmacy the same day.

Why the temperature matters

The storage rules sit in the Mounjaro instructions for use. Unused pens go in a fridge at 2C to 8C. A pen must never freeze, and a pen which has frozen must be thrown away. Once you have started a pen you can keep it at room temperature up to 30C, and it must go 30 days after first use.

The leaflet gives no out-of-fridge allowance for a pen you have not started. Keep it refrigerated until you start it. This is why delivery packaging is built around the 2C to 8C range, and why a warm parcel is a real problem, not a matter of taste.

Freezing is the less obvious risk. Cooling packs placed straight against a pen, a parcel left out on a frosty night, or a van parked overnight in winter can all freeze the medicine. Tirzepatide is a peptide medicine, and freezing can harm it without any visible change. This is why a frozen pen goes in the bin even if it looks normal.

What the pharmacy has to do

Online pharmacies in Great Britain work under guidance from the General Pharmaceutical Council (GPhC), the regulator for pharmacies. The February 2025 guidance on services at a distance sets out what the pharmacy must think through before sending you a fridge medicine:

  • delivery methods, timescales and dispatch are assessed, with refrigerated medicines named as an example
  • packaging is assessed for suitability, including temperature control and tamper evidence
  • the package is tracked and monitored to make sure it reaches the right person
  • the courier's terms and restrictions are checked, and the pharmacy stays responsible even when a third party delivers
  • records show the delivery method, the dispatch date and successful delivery
  • people are told about delays or disruption

The same guidance requires the pharmacy's GPhC registration number to be displayed on its website. You can check it on the GPhC register. For weight loss medicines, the prescriber must verify your weight, height or BMI independently, for example by video, in person or through your records. A phone call alone is not enough.

The usual delivery choices

Options depend on the pharmacy, so check before you order.

Signed delivery is the safer default. A parcel left in a porch in July can pass 30C within hours. A parcel left out in January can drop below zero overnight.

Parcel lockers and pick-up points are often unsuitable. Many lockers sit outside or in unheated spaces, and pick-up shops have no fridge for your parcel. Ask the pharmacy whether they allow either option for a fridge medicine. Many do not.

Plan your order

Order your next pen when you take the third dose from your current one. This gives you a week of slack for prescription checks, dispatch and delivery. The guide on how long a Mounjaro prescription takes explains the checks behind the wait.

  • Check the dispatch cut-off time and which days the pharmacy sends fridge items.
  • Avoid delivery on a day nobody is home.
  • Give a phone number and email for courier updates, and watch the tracking.
  • Allow more time near bank holidays. The Christmas delivery guide covers the busiest stretch of the year.

In summer, a morning delivery means less time in a hot van. The page on Mounjaro in hot weather has more on heat and storage.

When the parcel arrives

Open it straight away, not at the end of the day. Work through these checks:

  1. The outer box is intact, with no crushing, tears or signs of tampering.
  2. The cooling packs still feel cold. Soft, warm packs at room temperature are a warning sign.
  3. The pens are not frozen. Look for ice in the box, a pen stuck to a pack, or ice crystals in the liquid.
  4. The pharmacy label carries your name, the medicine, the strength and the instructions.
  5. The pen strength matches your prescription. Each strength has its own colour on the label.
  6. The expiry date has not passed, and a patient leaflet is in the box.

Then put the pens on a middle shelf of your fridge, away from the back wall and the freezer compartment. Keep them in the outer box to protect them from light.

If the parcel is warm, frozen, damaged or wrong

Do not inject from the pen. Put it in the fridge for the moment so nothing gets worse, and gather evidence:

  • photograph the box, the cooling packs, the pen and the label
  • note the time of delivery and any tracking messages
  • keep the packaging

Contact the pharmacy the same day. The pharmacist decides whether the pen is usable. A pen which has frozen must be thrown away under the leaflet. The pharmacy should arrange a replacement where the fault was in transit.

If the pen looks wrong in other ways, such as a strange label, missing leaflet, foreign-language packaging or a strength you did not order, treat it as a possible fake. The page on fake Mounjaro pens lists the signs. Do not use it, and report it through the MHRA Yellow Card scheme.

If the parcel is late

A late parcel rarely means a missed dose. The NHS advice on missed doses gives you up to 4 days after your usual day. If 4 days or less have passed, inject as soon as the pen arrives and carry on from your usual day. If more than 4 days pass, skip that dose and take the next one as normal. Never take two doses within 3 days of each other.

Chase the pharmacy, not only the courier. The pharmacy holds the contract with the courier, and GPhC guidance makes the pharmacy responsible for monitoring the delivery.

If you keep running into delivery problems, the guide on switching pharmacy on Mounjaro covers changing supplier without a gap in treatment. For buying safely from the start, see how to buy Mounjaro in the UK.

Questions people ask

How long can Mounjaro stay in the delivery box?

Only as long as the cooling packs keep the inside at 2C to 8C. The packaging is designed for the planned journey time, not for days on a doorstep. Put the pens in your fridge as soon as the parcel arrives and contact the pharmacy if the packs are no longer cold.

What should I do if my Mounjaro arrives warm?

Do not use the pen. Put it in the fridge for now, photograph the parcel, the packs and the pen, and contact the pharmacy the same day. The pharmacy decides whether the pen is safe and arranges a replacement if needed.

Can Mounjaro be left in a safe place?

Avoid it. A porch, bin store or doorstep can be hot in summer and below freezing in winter. Choose a delivery you sign for, have it sent to a workplace, or ask whether the pharmacy offers collection.

What if my delivery is delayed and I am due a dose?

You have time. The missed dose rule lets you inject within 4 days (96 hours) of your usual day. If more than 4 days pass, skip that dose and take the next one on your normal day. Never take two doses within 3 days of each other.

How do I know the pharmacy sending my pens is genuine?

GPhC guidance requires online pharmacies to display their registration number. Check the number and the pharmacy's address on the GPhC register. A UK pharmacy should also need a prescription, an assessment and checks of your weight and height before supplying.
