In short
- Needles are not included in the Mounjaro KwikPen pack. You need pen needles and a sharps bin before your first dose
- Use a new needle for every injection. Each KwikPen gives 4 weekly doses, so expect 4 needles per pen
- Put the needle into the sharps bin right away after the injection
- Fill a bin no more than three quarters full, never past the fill line, and do not recycle it
- After a needlestick from someone else's needle, wash it, cover it and go to A&E the same day
Every Mounjaro injection needs a new pen needle, and every used needle goes straight into a yellow sharps bin. The needles are not in the KwikPen box, so you need pen needles and a bin before your first dose. Your supplier may send both, or a pharmacy can sell them. When the bin reaches its fill line, seal it and take it to a pharmacy that accepts sharps, or book a collection with your council.
Why each needle is used once
The UK instructions for use tell you to use a new pen needle for each injection, to help prevent infections and blocked needles. Pen needles are thin and finely pointed. After one pass through the skin the tip bends and blunts, and the next injection hurts more and bruises more easily. A used needle also carries bacteria from your skin.
There is a pen reason too. A needle left on between doses gives air a path into the cartridge and medicine a path out. Over a week this can affect the dose. Take the needle off after each injection, not before the next one.
Each KwikPen gives 4 weekly doses, so you use 4 needles per pen. Keep a few spare in case one is dropped or bent while you attach it.
Getting pen needles
Mounjaro KwikPens take standard screw-on pen needles. Private suppliers often include a pack with your order. If yours does not, a pharmacy can sell you a box. Ask the pharmacist which needle lengths fit the KwikPen. Shorter needles suit most people, and the KwikPen how-to page shows how to attach one.
If you get sore, bruised or lumpy skin at the injection site, check you are changing the needle every week and rotating sites. The page on injection site reactions covers what is normal and what needs a check.
Getting a sharps bin
A sharps bin is a rigid yellow plastic container with a one-way opening and a lid you lock shut when full. The plastic resists needles, so a stray point cannot poke through the side.
You have three usual sources:
- your private supplier, who may send a small bin with your first order or on request
- a pharmacy, which can sell one over the counter
- your GP practice, which prescribes bins for many people who inject medicines
Kent Community Health NHS Foundation Trust confirms sharps bins are available on prescription from your GP or to buy from a pharmacy. GP practices tend to prescribe bins alongside medicines they also prescribe, so if you buy Mounjaro privately, plan to get your bin from your supplier or a pharmacy.
A 1 litre bin holds a few months of weekly needles. If you also put finished KwikPens in the bin, choose a larger size, since a pen takes up far more room than a needle. The KwikPen disposal guide covers the pen itself.
Using the bin day to day
Put the needle in the bin straight after the injection. The instructions for use say to do this right away. Do not leave it on the side to deal with later.
- Assemble the bin with the lid clicked firmly onto the base before first use.
- Keep the temporary closure shut between injections.
- Store the bin upright, out of reach of children and pets, away from heat.
- Do not push or shake items down to make room.
- Do not put your fingers into the opening.
- Fill to no more than three quarters, and never past the line marked on the side.
Kent Community Health NHS Foundation Trust also advises against bending, breaking or recapping needles, or putting sharps in other containers such as drink cans.
When the bin is full
Once the contents reach the fill line, lock the lid. On most bins this is a final closure which cannot be reopened. Write your name and the date on the label if there is one. Then store the sealed bin somewhere safe until it goes.
Pharmacy return
Some pharmacies take sealed sharps bins back from the public, and some do not. Arrangements differ between areas of the UK. Call ahead or ask at the counter before you carry a bin in.
Council collection
Many councils run a home collection for clinical waste and sharps. Buckinghamshire Council, for example, lets you book a collection of sharps boxes from your home online. Other councils ask for a GP referral before the first collection, some charge a fee, and most ask you to leave the locked bin in a set place. Search your council website for "clinical waste" or "sharps collection" to find the local rules.
Do not put a full or part-full bin in your household waste. Do not put a bin in recycling, even though it is plastic. The instructions for use state you should not recycle a used sharps container.
If you have no bin yet
Sort the bin out before injection day. A pharmacy can usually sell you one the same day.
If you have already injected and have nowhere safe for the needle, do not throw it away loose. As a short stopgap for one or two needles, drop the needle into a rigid plastic bottle with a screw-top lid, such as an empty laundry liquid bottle. Close it tightly, label it "sharps, do not recycle", and keep it out of reach. Do not tip the needles out later. Take the whole container to a pharmacy or ask your council how to hand it over. Get a proper sharps bin before your next dose.
Sharps on the move
If you travel over an injection day, take a small travel sharps bin in your hand luggage. Fill it during the trip and bring it home with you. Do not leave needles in hotel bins or aircraft toilets, where cleaning staff can be injured. The guide to travelling abroad with Mounjaro covers airport security and carrying needles on a plane.
Needlestick first aid
A needlestick means a used needle has pierced someone's skin. What to do next depends on whose needle it was.
| Situation | First aid | Then |
|---|---|---|
| You pricked yourself with the needle from your own injection | Let it bleed, wash with soap and running water, cover with a plaster | No further action needed unless it becomes red, hot or swollen |
| Someone else was pricked by your used needle | Same first aid straight away | Go to A&E the same day. Take the details of your medicine |
| You were pricked by a needle you did not use, or one of unknown origin | Same first aid straight away | Go to A&E the same day, as soon as you can |
East of England Ambulance Service NHS Trust guidance for needlestick injuries sets out the first aid steps: encourage the wound to bleed, wash it with soap and running water, dry it and cover it with a waterproof plaster. Do not scrub the wound and do not suck it.
The concern with someone else's needle is blood-borne infection, such as hepatitis B, hepatitis C and HIV. Treatment to lower the risk works best when started early, which is why A&E the same day matters. A child who finds a used needle at home falls into this group too, even if the needle was yours, because the hospital will want to assess them.
If the fear of needles makes injecting hard, the page on fear of injecting has practical ways to make it easier.
Questions people ask
Do Mounjaro pens come with needles?
No. The UK instructions for use state needles are not included in the pack. Your supplier usually sends compatible pen needles with your order, or you can buy them from a pharmacy. Ask the pharmacist which needles fit the KwikPen if you are unsure.
Can I reuse a Mounjaro needle?
No. The instructions for use say to use a new pen needle for each injection to help prevent infections and blocked needles. A used needle is blunter, may carry bacteria from your skin and can let air into the pen.
Where can I get a sharps bin for Mounjaro needles?
Your private supplier may send one with your first order. Pharmacies sell them, and some GP practices prescribe them for people who inject medicines. Small travel-size bins also exist for trips away.
What do I do with a full sharps bin?
Lock the lid so it cannot reopen, then ask a local pharmacy whether it accepts full bins or book a clinical waste collection with your council. Never put a sealed or part-full bin in household waste or recycling.
What if I prick myself with a used needle?
If it is your own needle from your own injection, the infection risk to you is low. Let it bleed, wash it with soap and running water and cover it. If someone else is pricked by your needle, or you are pricked by a needle you did not use, go to A&E the same day.
