In short
- UK airport rules allow essential medicines and hypodermic syringes in hand luggage. Cooling gel packs are allowed too
- A started KwikPen can be kept at up to 30C for up to 30 days. An unstarted pen stays at 2C to 8C
- A frozen pen must be thrown away, so do not pack pens in the hold or against ice
- Missed dose rule: inject within 4 days (96 hours) of the usual day, otherwise skip it
- Keep at least 3 days (72 hours) between two doses if you move your injection day
- Check the medicine rules of the country you are visiting with its embassy before you book
You can fly with Mounjaro. Pack every pen in your hand luggage, in its labelled box, with a copy of your prescription or a letter from your prescriber. A started KwikPen can stay out of the fridge at up to 30C for up to 30 days, so a short trip on one pen is simple. Unstarted pens need to stay at 2C to 8C, which means a cool bag with gel packs and a fridge at the other end. Before you book, check the destination's rules on bringing in prescription medicines.
Plan the trip around your pen
Start with a count. Each KwikPen gives 4 weekly doses and must be thrown away 30 days after first use. Look at your departure date, your return date and the date you started your current pen.
| Trip | What to pack |
|---|---|
| Under 30 days, current pen covers it | The started pen, needles, travel sharps bin |
| Current pen runs out during the trip | Started pen plus one unstarted pen in a cool bag |
| Over 30 days | Enough unstarted pens for the whole trip, a cool bag and a fridge at your accommodation |
TravelHealthPro, the UK travel health service run by the National Travel Health Network and Centre, advises carrying enough medicine for the whole trip plus extra for delays. Order your next supply early. Private orders can take several days, and the Christmas delivery guide covers ordering around bank holidays.
Do not plan to buy Mounjaro abroad. TravelHealthPro warns about the risk of falsified medicines overseas, and the MHRA has warned about fake weight loss pens. The page on fake Mounjaro pens explains the signs.
Keeping pens at the right temperature
The storage rules come from the Mounjaro instructions for use. Store unused pens in a fridge at 2C to 8C. Do not freeze a pen, and throw it away if it has been frozen. After your first injection from a pen, you can keep it at room temperature up to 30C, and you must dispose of it 30 days after first use.
The leaflet gives no out-of-fridge allowance for a pen you have not started. Keep it refrigerated until you start it.
Packing a cool bag
- Use an insulated medicine travel bag with cooling gel packs.
- Wrap the pens in their box or a cloth so they never touch a gel pack or ice directly.
- Pack the gel packs cold from the fridge, not frozen solid, if the bag will be closed for a short trip.
- Put unstarted pens in a fridge as soon as you arrive.
Hotel minibars and older fridges can run colder near the back and freeze a pen. Place the pens on a middle shelf, away from the cooling plate. If you can, put a small fridge thermometer next to them on the first night.
Hot destinations
A started pen is fine up to 30C. Above that, keep it in a cool bag, out of direct sun, and never leave it in a parked car or on a sunbed. The page on Mounjaro in hot weather covers heat and dehydration as well as storage.
Getting through airport security
The GOV.UK guidance on hand luggage at UK airports allows essential medicines and medical equipment needed for your journey. Hypodermic syringes are listed as allowed in hand and hold luggage. Cooling gel packs are allowed in hand luggage. For ice packs in hand luggage, the guidance says to contact the airport.
Each KwikPen holds less than 100ml, but security staff may still ask about the pens and needles. Make the check quick:
- Keep pens in their original labelled box.
- Carry a copy of your prescription or a letter from your prescriber.
- Pack pens, needles, sharps bin and paperwork together in one bag.
- Tell the security officer you are carrying an injectable medicine before your bag goes through.
TravelHealthPro advises contacting your airline in advance if you travel with injectable medicines. Airlines set their own rules on top of airport security, so check before you fly.
Keep every pen in your hand luggage, including spares. Hold temperatures are not controlled, and a bag can be delayed for days or lost.
The prescriber letter
A letter from your prescriber on headed paper is the most useful document you can carry. TravelHealthPro recommends one using the generic name of each medicine. Ask for a letter which states:
- your name and date of birth
- the medicine as tirzepatide (Mounjaro), its strength and weekly dose
- the pen needles and sharps bin you need with it
- the prescriber's name, registration and contact details
Online prescribers will often provide one on request. Keep a photo of it on your phone as a backup.
Countries with their own rules
Each country decides which medicines a visitor can bring in, how much, and with what paperwork. Some ask for a permit before arrival for some prescription medicines, and some limit you to a set number of days' supply. TravelHealthPro advises checking the status of your medicine with the embassy or consulate of the country you are visiting before you travel.
Check every country on your route, including any where you change planes. Do this before you book, since a permit can take weeks.
Time zones and your injection day
Mounjaro is a weekly injection, so a time difference of a few hours makes little difference. Take your dose on your usual day in local time.
Long trips or big time shifts may make your usual day awkward. The SmPC allows you to change the day of your weekly injection as long as at least 3 days (72 hours) separate two doses. After the change, carry on weekly from the new day.
If you miss a dose while away, the rule is the same as at home. The NHS advice is to inject as soon as you remember if it is 4 days or less since the dose was due. If more than 4 days have passed, skip it and take the next dose on your usual day. Do not give 2 injections to make up for a missed one.
Needles and sharps while you travel
Pack enough pen needles for each dose plus a few spares. Take a small travel sharps bin and bring it home with you. Do not leave used needles in hotel bins, where cleaning staff can be injured. The needle disposal guide covers bins and what to do with a full one.
Staying well while you are away
Stomach bugs are common on holiday, and vomiting or diarrhoea on Mounjaro can lead to dehydration. Learn the sick day rules before you go and pack oral rehydration sachets. Drink water through the day in hot weather, since a smaller appetite often means less fluid from food too.
If you need medical care abroad, show the doctor your prescriber letter so they know you take tirzepatide. This matters before any procedure with sedation or anaesthesia.
Travel insurance
Declare Mounjaro and the condition it treats when you buy travel insurance. If you do not, the insurer may refuse a claim linked to it. The guide to travel insurance on Mounjaro explains what to tell your insurer and how cover for lost or damaged pens works.
Questions people ask
Can I take Mounjaro pens on a plane?
Yes. UK hand luggage rules allow essential medicines and hypodermic syringes, and cooling gel packs are allowed in hand luggage. Carry the pens in their labelled box with a copy of your prescription or a letter from your prescriber, and show them at security if asked.
Should I put Mounjaro in my hold luggage?
No. Hold temperatures are not controlled, bags can be delayed or lost, and a pen that freezes must be thrown away. Keep every pen in your hand luggage.
How do I keep Mounjaro cold on holiday?
Use an insulated medicine cool bag with gel packs for unstarted pens, and put them in a fridge on arrival. Keep the pen from touching ice or a freezer pack directly. A started pen can stay out of the fridge at up to 30C for up to 30 days.
Do I need a letter from my doctor to travel with Mounjaro?
UK airports ask for proof of prescription for some medicines, and other countries may ask too. A letter from your prescriber naming tirzepatide, the dose and the need for needles is the safest option. A copy of your prescription is the minimum.
How do I take Mounjaro when changing time zones?
Stay on your usual day where you can, using local time. If you need to move your injection day, keep at least 3 days (72 hours) between two doses. If you miss a dose while travelling, take it within 4 days of the usual day, otherwise skip it.
