In short

  • A new component behind the plunger makes the plunger sit further down in an unused modified pen
  • After 4 full doses the modified pen has less leftover medicine, and the plunger sits near the bottom of the cartridge
  • Lilly says there are no differences in how the initial and modified pens work
  • Both pens hold priming volume plus 4 fixed weekly doses of 0.6ml, in all 6 strengths
  • Old and new pens are in circulation together until stock of the initial pen runs out
  • Lilly has not published a UK start date on its medical information page

Eli Lilly is bringing in a modified Mounjaro KwikPen in the UK. The change is inside the cartridge. A new part behind the plunger means the plunger starts lower in an unused pen, and less liquid is left once the 4 doses are used. Lilly says the two pens work the same way. Doses, strengths, injection steps and the number of doses per pen do not change, and you may receive either pen until the old stock runs out.

What Lilly has confirmed

The primary source for this change is Lilly UK Medical Information, which publishes a summary titled "Modified Mounjaro (tirzepatide) KwikPen design". It was last reviewed on 14 October 2025. The summary describes three differences, all linked to one new component.

Lilly's wording on function is direct: there are no differences in the functionality of the initial and modified pens.

What has not been confirmed

Lilly's medical information page gives no UK start date. It says the modified pens "will be gradually introduced into the market". Many online sellers have told customers the change starts in April 2026. We found no Lilly, MHRA or EMC notice stating this date, so we do not repeat it as fact.

The Lilly summary does not mention any change to the label, the carton, the colour coding, the dose knob, the needles or storage. If you read claims about a new dose counter or a different number of doses, check them against the sources listed on this page. We found nothing to support either.

The current patient leaflet on the EMC was last revised in August 2026. It still describes a pen with 4 doses and says a small amount of medicine may remain after all doses are given. This wording fits both designs.

What stays the same

Everything you do with the pen stays the same. Each Mounjaro KwikPen is a disposable, multi-dose pen for one person. It holds 4 weekly doses, plus the small amount used to prime the pen before each injection. The dose schedule does not change either. You start at 2.5mg for 4 weeks, then the dose rises in 2.5mg steps with at least 4 weeks on each dose.

For each injection you still:

  1. Check the pen label shows the right strength and has not passed its expiry date.
  2. Attach a new pen needle.
  3. Prime the pen as the instructions describe.
  4. Set the dose and inject into your stomach, thigh or upper arm.
  5. Remove and dispose of the needle in a sharps bin.

The guide on how to use the Mounjaro KwikPen covers each step in full.

The missed dose rule stays the same. Take a missed dose within 4 days (96 hours) of the usual time. After 4 days, skip it and take the next one on your usual day. Never take two doses within 3 days of each other.

Storage follows the patient leaflet for both designs. Keep pens in a fridge at 2C to 8C and do not freeze them. Keep a new pen refrigerated until you start it. After first use, a pen can stay out of the fridge at not above 30C for up to 30 days, after which you must throw it away.

The leftover liquid and the "golden dose"

The main practical effect is on leftover liquid. Some people have tried to draw an extra dose from an old-style pen once the 4 doses are used, often called a fifth or "golden" dose. The patient leaflet says not to try to use any remaining medicine. A partial dose drawn from the cartridge has no reliable strength, and using a syringe breaks the sterile system the pen relies on.

The modified pen leaves less liquid after the 4th dose, and the plunger ends near the bottom of the cartridge. Lilly says this makes clear the pen is finished and should be thrown away. The guide to the golden dose explains why the extra-dose practice is unsafe. If you worry about running out between pens, the page on the fifth dose question covers how to time your next order.

What to check on your next delivery

You may get an initial pen one month and a modified pen the next. Both are genuine. Use these checks when a new pack arrives.

  • Strength: the carton and pen label match the dose on your prescription.
  • Batch and expiry: printed on the carton and on the pen label, and the two agree.
  • Plunger position: higher in an initial pen, lower in a modified pen. Either is normal for a new, unused pen.
  • Liquid: clear and colourless to slightly yellow, with no cloudiness or particles.
  • Pen body: no cracks, and the dose knob turns as the instructions describe.
  • Cold chain: the pack arrived cold, and you put it straight in the fridge.

A lower plunger does not mean a pen is short of medicine. Lilly says the new component takes up space behind the plunger, and the pen still holds 4 full doses plus priming.

Telling a modified pen from a fake

Lilly's summary of the modified design was last reviewed in October 2025. Four months later, in February 2026, the MHRA warned about falsified 15mg pens. The two issues are separate. The fakes carried batch D873576 in the 15mg strength, and the warning sign was a dose knob which came off during use. A plunger in a different place was not part of the MHRA description.

Judge a pen on its batch, strength, label and how it works, not on plunger height alone. If anything looks wrong, do not use the pen and report it through the MHRA Yellow Card scheme. The page on fake Mounjaro pens and the MHRA alert explains the checks in full.

When to contact your pharmacy

Contact the pharmacy which supplied your pen if:

  • the strength on the label does not match your prescription
  • the pen looks damaged or the liquid looks cloudy or discoloured
  • the dose knob sticks, comes loose or does not click
  • the pen runs out before the 4th dose
  • the pack arrived warm and you are unsure how long it was out of the fridge

Disposal

Disposal does not change. Put used needles in a sharps bin straight after each injection. Once the 4th dose is used, dispose of the empty pen as your pharmacy or local council advises. The guide on disposing of Mounjaro KwikPens sets out the options in the UK.

Questions people ask

What has changed on the Mounjaro KwikPen?

A new component sits behind the plunger inside the cartridge. In an unused modified pen the plunger starts further down, and after the 4 doses less medicine is left over. Lilly says the pen works in exactly the same way as before.

When does the new Mounjaro pen arrive in the UK?

Lilly UK says the modified pen will be introduced gradually and gives no fixed date. Many sellers have reported April 2026 as the start. We were unable to confirm this date from Lilly, the MHRA or the EMC, so treat any single date with caution.

Is a pen with the plunger lower down a fake?

Not on its own. A lower plunger in an unused pen is the expected look of the modified design. Check the batch number, strength and expiry on the carton and pen label instead, and check the pharmacy is on the GPhC register.

Can I still get a fifth dose from the new pen?

No. Each pen is designed to give 4 doses, and the patient leaflet says not to try to use any remaining medicine. The modified pen has less leftover liquid by design, so there is less in the cartridge after the 4th dose.

Do I need to change how I inject?

No. Lilly says both sets of instructions for use describe identical dosing steps. You still attach a new needle, prime the pen, set the dose and inject once a week.

Does storage change with the modified pen?

Lilly's page on the modified pen does not mention any storage change. The current leaflet rules apply to both: keep the pen in a fridge at 2C to 8C, do not freeze it, and after first use it can stay out of the fridge at not above 30C for up to 30 days.
